Religious and Medical Accommodation Process for Students
Religious and medical accommodations for students may be requested for documented medical conditions and sincerely held religious beliefs.
Students with a medical condition or sincerely held religious belief must complete a Medical or Religious Accommodation Request.
The university may change, adjust, or modify determination or an accommodation at any time if there is a change in circumstances that warrants it.
Note: Students in programs that have their own requirements for students should follow the processes and deadlines of their academic department.
Medical Accommodation Requests
- Complete the Medical Accommodation Form (PDF).
- First, complete the student portion and print.
- Second, meet with a licensed healthcare provider to discuss your accommodation request and have them fill out and sign the form. The licensed health care provider can be a medical doctor (MD), doctor of osteopathic medicine (DO), advanced registered nurse practitioner (ARNP) or physician assistant (PA) licensed in the State of 熊猫在线视频.
- Submit the completed request form via email to the UAA Student Health and Counseling Center (SHCC).
- This information will be reviewed by the SHCC. Students will be notified by UAA email of the outcome of their accommodation request within five business days. If the decision is not possible in the five-day period, the university will inform the student in writing to their UAA email address.
- If your request for a medical accommodation is denied, you may request a reasonable accommodation for a disability through the UAA Disability Support Services Office.

Privacy Protections
Requests for medical and religious accommodations are treated in a confidential manner and will be stored separately from other student records.
Appeal Process
Students will receive written notice of the university鈥檚 decision concerning their request for accommodation. In instances where either the request is denied or the student disagrees with accommodations provided in the approval, the student may file a written appeal within five business days. Appeals should be directed to the UAA Dean of Students, and they will be considered by a three-person review panel comprising faculty and staff.
Students whose requests for a medical accommodation are denied may request a reasonable accommodation for a disability through the Disability Support Services Office.
